Premier West 2500 Overview

Deductible

Individuals choose to pay a little more, $2500 for the yearly deductible per person, because it lowers the monthly premium. After you've met your annual deductible, Premier-West 2500 shares the cost of medical care with you. We pay 80% of in-network service and you pay just 20%.

Office Visit Copayment

Once you've met your annual deductible you pay just 20% of the office visit. Your Premier-West 2500 plan picks up the rest.

Individual Out-of-Pocket Maximum

You'll be glad to know that your maximum out-of-pocket costs will never exceed $5,500 per individual. You'll receive a lifetime maximum benefit of $2,000,000 for all your health care needs.

Prescription Coverage

Prescription coverage is included in your major medical coverage. That means after you meet your annual deductible you pay only 20% of the cost for prescriptions. Your sign up packet will also list participating pharmacies and mail order information to receive even more discounts and reduce your costs.
