Deductible

We offer three options on the HSA plan for deductibles: $2,000 ind./$4,000 family; $3,000 ind./$6,000 family; $5,000 ind./$10,000 family.

Office Visit Copayment

After the annual deductible is met we pay 100%.

Individual Out-of-Pocket Maximum

You’ll be glad to know that your maximum out-of-pocket costs will never exceed the deductible plan you choose or the family cap associated with it. In fact, you’ll receive a lifetime maximum benefit of $2,000,000.

Preventive Care and Screening Services

Preventive care and screening services have been added to this benefit plan. You will receive well child care, breast cancer screenings, cervical, colorectal and prostate cancer screening along with cholesterol and lipid disorder screenings. These services are paid with deductible waived. See the Outline of Coverage for more details.

First Dollar Coverage Benefit

Deductible is waived for the first $500 of allowed charges for preventive and screening services.
