Guardian Overview

Deductible

We offer two unique Guardian Plans with two separate deductibles. Choose the low $500 in network deductible and $1,000 for out of network. Or, you can choose the $1,000 in network and $3,000 for out of network to lower monthly premiums. Remember, this plan has three levels of benefits which affect deductibles and coinsurance.

Office Visit Copayment

For those who choose the $500 in network deductible the copayment is 80/20 and the out of network copayment is 60/40.

For those who select the $1,000 in network deductible, the copayment is 60/40 and the out of network copayment is 50/50.

Individual Out-of-Pocket Maximum

Because the Guardian Plan offers three plans within a plan, the out of pocket depends on which deductible you’ve chosen. For example, a $500 in network deductible means you’ll only pay $2,000 and for out of network with a $1,000 deductible you’ll only pay $3,000. See the Outlines tab above to learn more.

Emergency Care

When you have a medical emergency, cost should be the last thing on your mind. There’s no guessing with Guardian. Depending on the deductible you choose and our three levels of benefits, you have a coinsurance payment that never changes. As low as 80/20 or minimally 50/50. First Dollar Health Care Coverage

Deductible is waived for the first $500 of allowed charges for office visits and urgent care with an in network provider. (Does not apply to mental health treatment and care received in the emergency room.)

Preventive Care and Screening Services

Preventive care and screening services have been added to this benefit plan. You will receive well child care, breast cancer screenings, cervical, colorectal and prostate cancer screening along with cholesterol and lipid disorder screenings.
