What services does my dental endorsement cover?

Generally, your dental endorsement covers preventive, basic, and major services as well as orthodontia. After the plan deductible is met, the above services apply coinsurance in the amount of 100% for preventive, 80% for basic, 50% for major, and 50% for orthodontia up to a maximum benefit of $1,000.

The maximum dental benefit per contract/calendar year is $1,000.

Do I have to use a participating dental provider?

No. You may use the dentist of your choice.

What services does my vision endorsement cover?

Your vision endorsement covers one comprehensive eye exam and/or refraction every 12 months for a $10 copayment when you use a participating VSP provider and a $100 hardware benefit.

Do I have to use a participating vision provider?

While there are advantages in using a participating VSP provider, you may use the provider of your choice. The benefit you receive depends on the status of the provider you select.

How does my hardware benefit work?

Customers are eligible to receive up to $100 to apply to hardware purchases every 12 months. Hardware includes prescription frames, lenses and contacts. This benefit is available to members every 12 months beginning from the date of the first service. Members are responsible for the hardware costs that are greater than the $100 benefit.
